Who can apply
You can apply if you:
- have a permanent and substantial disability that means you cannot walk or have considerable difficulty walking; or
- are registered as blind with EYECAN.
You can apply for yourself or as a carer on someone else’s behalf. The badge belongs to the person, so they can use it when travelling in different vehicles.
Before you start
You will need:
- the applicant’s name, date of birth and address
- their Jersey Social Security number, if they have one
- their GP surgery’s name, unless they are registered as blind with EYECAN
- a photograph of their current badge if renewing or requesting a replacement.
You will also be asked to take a clear photograph of the applicant’s face during the application. If you start on a computer, you can switch to a phone to take it.
What happens next
After you complete the form and pay, your application will be submitted for review.
If you are applying because of difficulty walking, a doctor at your named GP surgery will need to complete a medical certificate. This is not required if you are registered as blind with EYECAN.
You will then be told whether your application has been approved. Payment does not guarantee approval.
Replacement badges
A replacement costs £15 and expires on the same date as the original badge.
